A binary-coded switch is more efficient with IO. But I am confused by your description.
Do you mean:
So that B and D have identical outputs?

Do you mean:
| Position | OutA | OutB | OutC | OutD |
| A | 1 | 0 | 0 | 0 |
| B | 0 | 1 | 0 | 1 |
| C | 0 | 0 | 1 | 0 |
| D | 0 | 1 | 0 | 1 |
So that B and D have identical outputs?

That sounds wrong. (it makes the switch useless).
Perhaps there are some wires touching where they shouldn't or some other wiring fault?
