Manual toolchanger does not show up

I'm having a problem on a configuration that I made for a friend. Linuxcnc loads correctly, the first manual toolchanger window shows when hal_manualtoolchange loads and everything works correctly, except that the manual toolchange window does not show when a tool must be changed. When this happens Axis does not execute MDI commands until we power off and on Axis (F2).

Any thoughts of what might be the cause?

net tool-prepare-loopback iocontrol.0.tool-prepare => iocontrol.0.tool-prepared
