I am trying to determine if one of my Mesa 5I25 cards is bad.

I created a simple hal config that conencts a siggen clock output to hm2_5i25.0.gpio.000.out pin

I set hm2_5i25.0.gpio.000.is_output to true

If I run halmeter and look at the hm2_5i25.0.gpio.000.out signal it goes from true to false every 500ms.

Does not mean the actual IO pin on the DB25 should also me going high and low? It just stays at 0V.

Yes, if and only if the pin has not been claimed by a firmware module.

Are you using "num_stepgens=0, num_encoders=0" etc to turn all pins into GPIO?
