5i25 w7i76D 5i25 no firmware and no response to install one

I have verified my jumpers and power requirements on the 7i76.  5i25 w1 up, w2 up, w3 up, w4 up, w5 up.  7i76 w1 left, w2 left, w3 left, w4 right, w5 right, w6 right.  Seems like my 5i25 is dead.  Purchased these boards a week ago, and my linuxcnc & mesaflash install is from today.  Are there supposed to be LEDs on the 5i25 board?  I do not see any.

Try to load the .bit file:
rob@CNC:~$ sudo mesaflash --device 5i25 --write 5i25_7i76x2.bit

Error: BootSector is invalid

So tried this:
rob@CNC:~$ sudo mesaflash --device 5i25 --fix-boot-block --fallback --write 5i25_fallback.bit
Checking file... OK
  File type: BIT file
Erasing sector 0 boot block
BootBlock installed
Failed to write valid boot sector


Nothing seems to be happening, what can I do next?

Looks like a card access/connectivity issue.

I would try another slot or cleaning the PCI slot on the motherboard.

BTW if you have a card access issue (you cannot get a reasonable
mesaflash --readhmid response), its not a good idea to attempt to
write anything to the cards flash memory as you are likely to corrupt
the configuration image.

cleaned port, reinstalled card. tried readhmid and says No 5i25 board found.

OK first I would check if the card is configured.

If you have any solid red LEDs on after power up,
the card failed to configure and must be re-programmed via
JTAG to recover (This can be done by sending the card back to Mesa)

CR3 & CR4 are solid red as soon as computer turns on.

Yes, if the CPU cannot access the card properly you are going to have all
sorts of issues.

I would suspect the original issue was a dirty or bad PCI slot but it
could also have been a bad/marginal 5I25 so sending it back for
re-programming and retesting is a good idea.
