I suspect that this is a Gtk2 to Gtk3 transition problem.
Are you running the racktoolchange.ui from LinuxCNC 2.9.1 or a version from an older release (Note that I have no idea if anything has changed)

Can you check if you get the same problem running the rack toolchange sim config from the LinuxCNC install?
