Okay, with the Hal Meter here is what I am getting:
hm2_5i25.0.encoder.00.input-index - alternates between TRUE & FALSE
hm2_5i25.0.encoder.00.velocity - changes to a very small negative number -.00xxxxxxxxxx
Everything else on that encoder remains unchanged when I turn the handle.

No, it stayed negative the entire time and only went between -3.9v and -3.75 but not more or less. Is my polarity wrong? Should it be +3.9?

Do you get +3.9V at some positions?
if you ever get any close to 0V across the A and /A or B and /B you have a wrong connection

If you don get both + and - voltage across A and /A , B and /B and Z and /Z (and never 0V)
you have a mis-connect or open signal

I'm in! I have encoder Position and count on the Hal meter! I can even see movement on the preview window of linuxcnc when I turn the handle.
Unfortunately, i still have the joint access error whenever I try to move, however.
