A binary-coded switch is more efficient with IO. But I am confused by your description.

So that B and D have identical outputs?

when selecting a it turns yellow
when selecting b it turns yellow with d
when selecting c it turns yellow
when selecting d it turns d and b yellow

C | 0 | 0 | 1 | 0 |
D | 0 | 1 | 0 | 1 |
So that B and D have identical outputs?

Thats looks correct in your diagram i doublechecked.
Yes B and D are identical.

That sounds wrong. (it makes the switch useless).
Perhaps there are some wires touching where they shouldn't or some other wiring fault?
