I have a 7i97 and the PWM #3 + #4 + #5 does't work.
On #0 is the X axis,
on #1 is the Y axis,
on #2 is the X. And its work.
On #3 i have all the time +10v.
on #4 and 5 in no Voltage.

Note that PWM 4 and PWM 5 have separate enables and its possible that
these have not been set true. (PWM 0..3 have a common enable)
On PWM3, What are the PWM setup values?
halcmd show all | grep pwm.03
