So I/O access fails
So I really have no idea but looks like either a BIOS issue
(subtle bridge setup bug) or a 5I71 / Motherboard hardware problem
(BIOS bugs with bridge setup are quite common)

Very strange thing..
I tried the same config with the 5i71 in another pc and it worked fine .. so it seems that the intel d525mw has a problem with its pci port or something..
I also tried an bios update and loaded the standard configuration but that also doesnt change anything on that problem
Seems I have to buy a new board or use an other pc

This is ringing vague bells. Don't you get this when the card has a jumper in set-up mode? Or is this the "dirty PCI slot" one?
I recall the error reported not matching the actual cause, I know that much.

OK thats a different error now. The first error was
hm2_4i65.0: FPGA did not reset: /INIT = 1, DONE = 1
hm2/hm2_4i65.0: failed to reset fpga, aborting hm2_register
