Andy,

Ref: encoder.N.missing-teeth

My encoder disc has 100 slots. Should I blank out two slots and use this method?
Do we leave the 'scale' still set at 100 ?

Yes, the scale still stays at however many teeth there would be. 
You can blank out one or more teeth.

I think that I would start with tape if it is an optical sensor. If it is inductive then neither tape nor epoxy will work. 

Missing-tooth index is, indeed, only available with 2.9. (though it would work with 2.8 if you compiled it for 2.8) 

Setup is as simple as connecting just channel-A and using a "setp" in the HAL file to set the number of missing teeth.

Does this look correct?

Swap:
net pulse-index-combined parport.1.pin-10-in encoder.0.phase-A encoder.0.phase-Z

for
net pulse-index-combined parport.1.pin-10-in encoder.0.phase-A
setp encoder.0.missing-teeth 2
