Andy,
Ref: encoder.N.missing-teeth
My encoder disc has 100 slots. Should I blank out two slots and use this method?
Do we leave the 'scale' still set at 100 ?

You can blank out one or more teeth.

Yes, the scale still stays at however many teeth there would be.My encoder disc has 100 slots. Should I blank out two slots and use this method?

My thoughts were to blank out two teeth with epoxy, but I'm unsure how to set up this 'missing-teeth'. Is this only available to ver2.9 ?

Missing-tooth index is, indeed, only available with 2.9. (though it would work with 2.8 if you compiled it for 2.8)
Setup is as simple as connecting just channel-A and using a "setp" in the HAL file to set the number of missing teeth.

I think that I would start with tape if it is an optical sensor. If it is inductive then neither tape nor epoxy will work.My thoughts were to blank out two teeth with epoxy, but I'm unsure how to set up this 'missing-teeth'. Is this only available to ver2.9 ?

Does this look correct?
Swap:
net pulse-index-combined parport.1.pin-10-in encoder.0.phase-A encoder.0.phase-Z
for
net pulse-index-combined parport.1.pin-10-in encoder.0.phase-A
setp encoder.0.missing-teeth 2

Hmmm, I think I've done something wrong
