Try a lower spindle speed. It is possible that the encoder counter can't keep up, or that the steppers are maxjng out on speed.

Update:
1. Thread pitch not matching with 2 mm pitch. I am running threading macro G76.
2. Have altered RPM
@ 200 RPM pitch comes out 3.2 mm
@ 400 RPM pitch becomes fine 1.6 mm.
3. Z axis can accelerate to 6500 mm/min.
What else I have to look for proper thread.

I have 1:1 gearing from my motor to my spindle.
I had two issues with my system.
1st was unreliable encoder and wiring that was loosing counts due to encoder arrangement and wiring etc.
To resolve this I used the Servos built in Quadrature encoder.
2nd was the inability of the LPT port to count the pulses reliably.
This was resolved partially by turning down the resolution of the built-in encoder to 16 pulses
(16x4=64 in HAL), but the LPT port still struggles to cope above 500 rpm
I should have opted for a Mesa card.

Everything looks OK as far as encoder count rate, maximum step rate and axis velocity limits is concerned.
I am beginning to wonder if there is something like a slow optocoupler gettting in the way.
If you monitor the spindle velocity from the encoder with the halmeter, does it keep up as you increase the spindle speed?
(It will bounce around quite a lot, just see if it continues to be about right at each spindle speed, or whether it maxes out at around 200 rpm.)

Sir, Currently I have a Stainless Steel disk with 100 slots. Maybe there are some reflection issues. I have ordered a laser cut disk MS with 50 slots. Will update the results soon....

Extra pulses would make the Z axis move too fast.
Your problem is that the carriage is moving too slowly.
So, I am wondering if something in the signal path is too slow.
For some reason you seem to be losing encoder pulses, the system thinks that the spindle is moving more slowly than it actually is, when the speed is higher.

1. I have installed MS sheet 2 mm thick, 50 laser sloted disc with 1 Z index.
2. Installed Ball screw for extra precision.
still same result, I am not being able to cut proper thread
tested with 200, 300, 400, 500, 600, RPM.

I downloaded and compiled latest 2.9.0~pre0 version but the encoder.c component inside it, has no pin missing-teeth.
