Well I am not asleep.
Just as a matter of interest what versions of sheetcam are you using as the dev version gives different options.
where you can start at hole centre with a plasma tool

I believe that is the dev version that I have just downloaded
This is only in the dev version for plasma and not in the standard version

I've tried it and it appears to work in that it creates G-code and sheet Cam selects the Hole centers in a plasma jet operation and now we are all singing from the same hymn sheet it makes more sense.!
However, I think that possibly "Pierce only" could be broke.? as it still doesn't work. I've tried the attached file using both the Pierce button and directly putting the command #<pierce-only> = 1 into the G-code and it just cuts the Job as normal.
Attached my Config and the G-code file.
